How to Find Your Form 1095-A Online
If you had health insurance through the Marketplace (HealthCare.gov), you'll need Form 1095-A to file your taxes. Here's how to find and download it.
What is Form 1095-A?
Form 1095-A, Health Insurance Marketplace Statement, is a tax form that shows the months you and your family members had health coverage through the Marketplace, the amount of premium paid, and any advance premium tax credit (APTC) you received.
Why You Need It
- • Required to complete Form 8962 (Premium Tax Credit)
- • Reconcile advance credits received during the year
- • Claim additional premium tax credit if eligible
- • May affect your refund or amount owed
When to Expect It
- ✓ Available by January 31 each year
- ✓ May arrive by mail (if you opted in)
- ✓ Always available in your HealthCare.gov account
- ✓ Covers the previous calendar year
Important: You must file Form 8962 with your tax return if you or anyone in your household enrolled in Marketplace coverage and received advance payments of the premium tax credit. Without your 1095-A, you cannot complete this form.
Step-by-Step: Download Your 1095-A from HealthCare.gov
Log In and Select Your 2025 Application
Log in to your Marketplace account at HealthCare.gov and select your 2025 application — not your 2026 application.
Important: Make sure to select the 2025 application for your 2025 tax year 1095-A, not the 2026 application.
Select "Tax Forms" from the Menu
Once you're in your 2025 application, look for "Tax Forms" in the menu on the left side of the page and click on it.
Download Your 1095-A PDF
Under "Your Form 1095-A", select "Download PDF". If a form has a "Corrected" status, download the corrected form instead of a previous version. After downloading, save the PDF to your computer.
Tip: If you see a "Corrected" label on any form, always use the corrected version for your tax filing.
Print a Copy for Your Records
Open your saved file. Select "File" in the top left, and "Print" to print a copy for your records.
Success! Keep this form with your tax documents. You'll need the information from Parts I, II, and III to complete Form 8962.
Have more than one 1095-A?
You may have more than one Form 1095-A if:
- • Your household enrolled in more than one Marketplace health plan
- • You reported a life change during the year
If the Marketplace-assigned policy number in Box 2 is the same for all forms, use the most recent form.

Important Tips
Check All Household Members
Verify that all family members who were covered are listed on the form. Each person covered should appear in Part I of the 1095-A.
Verify Coverage Dates
Make sure the months of coverage shown in Part III match the months you actually had Marketplace insurance. Report any discrepancies to HealthCare.gov.
Contact Support if Needed
If your form is missing, incorrect, or you need a corrected 1095-A, contact the Marketplace Call Center at 1-800-318-2596 (TTY: 1-855-889-4325).
Keep a Copy
Save a copy of your 1095-A with your tax records. You should keep tax documents for at least 3 years in case of an IRS inquiry.
